To become appointed:

Click 'Next' at the bottom of each page to view all tabs
Upload required documents:
AHIP Cert (if needed)
FFM Cert (if needed)
Click on 'Submit to Health Plans'
You will be contacted when your appointment has been processed.
*Incomplete applications may cause delays in becoming appointed with Health First. You will be notified of an incomplete application and allowed five business days to submit missing or incomplete documentation. If the missing documentation is not received within five business days your application to become appointed will be denied. If you still wish to become appointed you must return to www.myhfhp.org/broker and submit a complete application.
Brokers that sell Individual and/or Medicare plans only, must carry a Florida resident license.
Brokers that sell Commercial Group, can carry a Florida resident or non-resident license.
Brokers must provide a valid copy of their Insurance license to Health First Health Plans upon request for appointment.
Brokers must provide a completed W9 form and keep a valid copy on file at all times with Health First Health Plans.
Brokers must carry Errors and Omissions liability insurance with limits of at least 1M per claim and 1M aggregate and keep a valid copy of the declaration page on file at all times with Health First Health Plans.
Brokers receiving commissions directly must complete the Health First Health Plans Direct Deposit process within 30 days of appointment or prior to selling business, whichever comes first.
Brokers selling Health First Health Plans and/or AdventHealth Advantage Plans products must complete the required Onboarding training. How to complete training will be relayed after your appointment is finalized.
Terminated Brokers requesting reappointment must contact Broker Services for business case and leadership consideration prior to reappointment.
